Getting Workflow notification error CHANGE Request in PO

Hi ,

One of my user has encountered below workflow notification error for change request in PO

Approval workflow database error occurs while approving Standard Purchase Order XXXXX

Contact your system administrator to identify the problem and to use SQL*Plus or Workflow Monitor to reactivate the approval workflow where it left off.

Give your system administrator the following information:

PL/SQL procedure: PO_ReqChangeRequestWF_PVT.Process_Cancel_Acceptance
Error: User-Defined Exception

I came across one Metalink note 415153.1 but it seems for 11i only. Do we have any metalink note for R12 or any other solution
