F4812H workfile history mass updated

On 3 occasions over the last 18 months we have had processes perform a mass update (approx 23 million records in the last instance) to the F4812H table un-intentionally. In each case the Journal Status was cleared out to blank, and the date and user ID in the audit fields were updated with the user that triggered this event. Initially this process was triggered by actions performed in the P4812, however the last instance was somehow triggered via the R48121 UBE. Has anyone experienced this issue?
