Insert query issue

Hi,
We are facing this strange issue in PROD database whereby, selection query below selects around 19000 records whereas insertion query just inserts 45 records. Any clues?
                                             
INSERT INTO PS_ICI_UATH_AD_STG (SETID 
 , DEPTID 
 , EMPLID 
 , EMPL_RCD 
 , NAME 
 , FIRST_NAME 
 , HIRE_DT 
 , COMPANY 
 , LOCATION 
 , GRADE 
 , POSITION_NBR 
 , DESCR 
 , DESCR1 
 , DUR 
 , PUNCH_DTTM 
 , SCHEDULE_ID 
 , DAYNUM 
 , OFFDAY_IND
