Locations screen does not show properly

Hi,

I took the application export of an application from an environment and import the same backup to two different environments. I import the application export and worked fine for the second environment but the third envronment it did not show any error messages however after login to the FDM web  portal --> metadata -> Location, I see the screen does not show correctly. Please see the attached screen shot.

Any ideas?
