Unique constraint error applying patch 13385260

Rel 12.1.3

We got error listed below while applying patch 13385260 on top of 12.1.3. Please note that query provided on readme of patch did not return any records and that we have applied pre-req patches 11819064 and 10254013 as listed on patch re-reqs list.
