Duplication in acitivty history

All,

We recently upgraded from Oracle 11.0.3 to Oracle 12.1.3 env. Users are repeatedly shown a dialog box after they submit requisition or PO. They simply need to click ok , ok , ok  and finally it would submit. Due to this we see lot of entry for submission at same time in activity history. Is this a bug? (snapshot attached).
