Please check useful advices about installation:
1. Ensure you are using latest rapidwiz version
The most current version of the Rapid Install wizard is 12.1.1.13
Check Note 798258.1 12.1.1 Oracle Applications Release Notes Release 12.1.1

2. Verify known issues and installation logs as per:

3. Be sure that your global inventory is OK. Ensure that your ORACLE_HOMEs are not already registered:

4. Ensure that your stage is correctly created:

5. Remove any previous failed installations:

6. Verify that all OS prerequisites are available in your system before starting the installation:

7. Understand and correctly identify what services should run on every server:
