How to identify the disk location in Storage

Hi,
Please help me in identifying the disk where it is located in the storage:We have 6 storage and each storage(Storagetek 2540) has 12 disk. Here is the output from the format command:
       0. c1t0d0 <DEFAULT cyl 36274 alt 2 hd 255 sec 63>
          /pci@0,0/pci10de,5d@c/pci1077,171@0/fp@0,0/disk@w202400a0b87449be,0
       1. c1t0d1 <SUN-LCSM100_F-0735-277.90GB>
