adpatch fails exit with error

Hi,I'm running the american patch u6678700.drv for upgrade from 11.5.10.2  to R12.1.1
the adpatch session exit with error
Would somebody help me  resolve this issue and continue with upgrade process
Please you can check the : u6678700 log file in attach
