Qlogic QLE2562 PCIe FC card firmware bug can cause data corruption resulting in ZFS checksum errors
Here are the Symptoms for this issue:
ZFS checksum errors occur on every reboot of a server.
When this issue is encountered ZFS checksum errors noted in FMA may span multiple LUNs and different offsets as shown below (zio_offset).
This issue will occur where the following hardware is installed:
QLogic QLE2562 PCI3.0 Fibre Channel card AND where there are LUNs > 2TB. Until now, this issue has only been seen on IBM x3850 servers with BIOS1.73 or higher.
Booting with a Linux kernel triggers the same corruption.
