US PAY: Employee was deducted 3 times the normal amount for Life Insurance and AD and D After Taxes

Hi,

One of the employees got deducted 3 times the normal amount for her Life Insurance and AD and D After Taxes. Does anyone know why this happened? The employee is a bi-weekly employee and this happened on the 24th payslip (there are 26 payslips in a year since she is a bi-weekly).
