cancel /hold stock request cause the wrong inventory balance

We use PS 8.8 SCM /FIN system , and we found sometime when user do 'cancel/hold stock request ' the system will cancel the stock request , but not release the qty reserved /picked in PS_DEMAND_PHYS_INV table , so cause the inventory balance is wrong , question

1- in what situation this issue will happen ?

2- Any hot fix for such issue ?
