Oracle Grid Control { Application Management Pack } trace SQL to form

I am using Oracle Grid Control { Application Management Pack } to monitor my ebus

database, its excellent and I have seen huge benefits

Example , The Tuning advisor advised us that Payslip SQL required tuning

so I had logged a SR and was extremely fortunate to get a developer who understood and

tuned the sql, now Payslips completes in less than 15 minutes, This benefit was not only for

me but for all South African Clients.

My challenge is that when tuning advisor picks up SQL which uses more than 80% cpu

and I logged it with Oracle Support they asked me which form / Self Service page is affected.
