User appears in organization that is not assigned to him

User SHUSSAIN could see that a work order is completed for organization DDT, although he has no access to this organization at all. He only has access to DDF.Issue is not reproduced and not currently faced.
A work order is completed in the DDT organization, but is saved as SHUSSAIN who is altogether from a different org (DDF) and who has no access to the DDT work orders at all. Customer is on 11.5.10.

How did this happen? What could be the cause of this?
