<< rvtptcontrol failed >> Inspection creates transfer transaction

Dear Friends,
While inspecting internal Requisitions, we get message 'rvtptcontrol failed' and 'Transfer' transaction is getting created instead of 'Accept'.
Any idea why this happens and help to fix this issue. (R12.0.4)
