Absence Management Module: While creating absences; contact address and phone number during leave pe

Hello,

Please when creating absences, we would want to include contact address as a field and phone number as another field on the create absences form.

This is to ensure staff enters the address of where they will be during leave as well as the phone number they can be reached during leave.
