Why does error ORA-04030 occur on different instances intermittently after installing an OS patch?

ORA-04030: out of process memory when trying to allocate 254488 bytes (QERHJ hash-joi,kllcqas:kllsltba)
ORA-04030: out of process memory when trying to allocate 64544 bytes (sort subheap,sort key)

Following have been checked and they are not a problem:

- physical memory
- swap space,
- OS limits, and
- memory leak
