R078500 (941 Worksheet) Additional Medicare Tax calculating wrong

FYI.

We are on Apps 9.0, Tools 9.1.0.3.  I applied ESU JL21053 for the fix for the Additional Medicare Tax on the 941 Worksheet (R078500).  On the Totals page it appears that the $200,000 exemption for each EE is only taken once per the entire company.  Thus overstating (by a huge amount) the Calculated Amounts for (941). 

I created SAR 3-7010178771 for this.
