PO Approval Workflow error - ORA-06512: at line, Get PO Attributes error.

We have several PO's which are stuck in the PO approval workflow with a ORA-06512: at line, Get PO Attributes error.  Any idea how to resolve?
We tried the Retry Errored Workflow Activities concurrent program but this does not resolve the issue.

We recently upgraded from 11i to R12.1.3 but requested that all PO's be approved prior to the upgrade.  These PO's have since been modified and require re-approval but are stuck 'In-Process'.
