VCP 12.1.3.8 patch 14247039 : Failing with ORA-06512: at "SYSTEM.AD_DDL"

When applying VCP 12.1.3.8 patch # 14247039 (using adpatch) on a 12.1.3 instance getting below error (Worker failed)

Run AutoPatch R120 MSCSTEXG.sql FAILED

sqlplus -s APPS/***** @/u01/apps/HRDEV/apps_st/appl/msc/12.0.0/patch/115/sql/MSCSTEXG.sql
Connected.
