Hi There,
I created this post to discuss

Offline agent patching fails with Exception while initializing: OPatch patch ID is not correct (Doc ID 1543329.1)

This issue could be happen if you like to patch targets in offline mode.
If the opatch version is not updated you have to update opatch as described int he note
