IBY_0002 errors

Hi,

We are experiencing intermittent issues with credit card processing when we book the order.  Oracle returns a message indicating the order has been booked, but the "Book Order" button is not grayed out.  When it (Book Order) is pressed again, Oracle returns an IBY_0002 error for a Duplicate Id.  At this point our only recourse is to create a new order (which books and authorizes correctly) and cancel the original order.

Can anyone advise what is causing this, and suggest a remedy for the issue?  To date, I have not been able to find anything on Oracle Support that addresses this issue.
