ORA-07445 and ORA-00600 alerts

A number of our 11gR1 instances have recently started generating the following entries in the alert log.

ORA-00600 arguments: [17113], [0x000000000] together with ORA-07445 ACCESS_VIOLATION "UNABLE_TO_READ" as well as arguments including ksedmp and kgh_heap_sizes:ds.

We have also seen occurences of UNABLE_TO_WRITE where the actual database goes offline as a result.

The trace files do not provide much information and I believe this is related to a performance problem - SGA/PGA sizing as ADDM does show some issues with User I/O wait events and Average time spent in User I/O wait per execution was 63 seconds.
