Create Accounting is run automatically

Hi all,
We are facing an issue that Create Accounting & its subsequent request(s) is run against a user id; but actually its not run by him. On history - System Logon:  UNKNOWN.

First we ignored; but the said issue is faced several times. Need clarification that is it a bug or is run against any activity(s) performed by the said user.

Further user is also unable to view the output; as the output is XML code.

Please guide.
