Short Item number 0 2nd item number blank

Hi

Just want to know if you have seen this before where Short Item number 0 2nd item number blank, Apart from that no audit field was updated like User ID, Program ID etc.Due to this issue we could not create the PO with blank item number and N line type.It is giving the error message Item branch does not exist,It is just one item we have work around remove that item from table and it will work but I am curious to know how this could happen?
