RAC upgrade from 11.2.0.2 to 11.2.0.3 failure

 Hello,
Not sure if anyone in the community encountered this issue but I have never been able to successfully upgrade a RAC from version 11.2.0.2 to 11.2.0.3. I believe I have the correct procedures  including applying patch 12539000. However, the rootcrs.pl -upgrade script fails everytime I attempt to do it for one reason or the other. I am performing a rolling upgrade.

Here is the latest error I encountered
