Regd Moving to new architecture in EBS R12.1.3

Hi
I need noteid's and documents ...or procedures to move existing architecture to Multinode(Separate servers)
Present we are having one DB and one MT node and we need to convert to Two node DB(RAC with ASM ) and Two MT( shared application filesystem ) and to enable PCP
and implement Loadbalancer for apps EBS R12.1.3  Platform is Linux

And also  need noteid  to upgrade db from 11.2.0.2  to 11.2.0.3 and also CRS upgrade from 11.2.0.2  to 11.2.0.3
