Emulex HBA Firmware?

 Hello Everyone,
I'm having a problem with HBAs in an M4000 servers. The servers are split into hardware domains one of which require boot from SAN. Of the seven HBAs we've received from our vendor only one is able to boot from SAN. Physically they all appear to be identical SG-XPCIE2FC-EM8-Z HBAs.  I think (hope) its related to firmware although I think all the firmware is up to date. Attempts to update the FCode appear successful but as the output shows one card has no Fcode. Below is output from 3 of the HBAs. One boots from SAN the other two won't.
