Notes lost from email center messages

Message is in the queue and supervisor adds notes (open the message and click "Add Note" and apply).
Later the messgae is assigned to agent, when the agent opens the mssage and goes to add note, prior notes added are LOST.

Unable to find the root cause as the issue is not replicable at will every time. Seems flaky. Need assistance on below.

I got the notes table JTF_NOTES_B and JTF_NOTES_TL but how to link with email message?

Any additional suggestions most welcome.
