BI Publisher RTF Output . RTF template not picking up header and footer setting correctly

Hi Experts,

Issue with Rtf Output letter, 3 pages long, has different header and footer on first page.
Once this is run through BI publisher the implicit page breaks are ignored and the first page header/footer appears on the first three pages,
ie until it gets to the explicit page break at the end of the group. 

 Context : BI Publisher 5.6.3 on the e-business suite 12.1.3
Applied patch 7487412 and 9905140:R12.XDO.B but issue still occurs .

 It is a bug??

 Issue can be reproduced using attached files and BIP Desktop ( output must be RTF ) :
