Report Parameter LOVs missing for field Type

We are trying to implement our parameterzied report By Sales Rep and the  Report Parameter Definition Applet > Report Parameter Definition Controls > Ctrl Type Cd has a picklist on it called Report Param Ctrl Type, however the LOVs are missing for this picklist causing us not to be able to select values from the field in theapplication.  Does anyone have those values or can tell me why they are missing?
