ORA-07445: exception encountered: core dump [kgldpo0()+1876] [SIGSEGV] while R12 upgrade

While running R12.1.1 upgrade driver on 11.2.0.3 database, adworks fail to execute files OEXUSETS.pls, OEXULINS.pls and OEXVRTOS.pls.

Getting below error while trying excuting these manually.

SQL> @/eas/app/APSVCP/R12/12i/ont/12.0.0/patch/115/sql/OEXUSETS.pls
CREATE or REPLACE PACKAGE OE_Set_Util AS

ERROR at line 1:
ORA-03113: end-of-file on communication channel
Process ID: 23495
Session ID: 1023 Serial number: 5

ERROR:
ORA-03114: not connected to ORACLE

Alert log shows below error.

Please help me resolving this issue.
