Merge patch error unable to flush log file

Release 12.1.3

We have experienced an error when tryinig to apply a merge patch. The patch is stating that it is unable to flush the log file, thus, is not allowing the patch to compete. Please see below text as well as the two attached documents for further details.

AutoPatch error: Unable to close log file

[appltrnd@db3324]$ tail -23 TRND_u_merged_1.log
context varchar2(80) not null, pdi_product varchar2(10) not null,
pdi_username varchar2(32) not null, command varchar2(30) not null,
file_product varchar2(10) not null, subdirectory varchar2(250) not null,
