12.1.3.6 Component Removal/Installation History Report

We have uptaken RUP6 in a test environment and have had success running the reports with a small set of test data, but have not had any success with the   Component Removal/Installation History Report. We have reviewed the TOI from 2011 and it mentions some things about a Quality Plan needing an element called "Certification Type". Not sure if this is still required and if so can more detail be supplied on this setup. Any other tips would be appreciated as well.
