11i: Patch 14182626 - Fails Processing b5402149a.sql and b5081398a.sql

Hi,
Installing patch 14182626 caused the following:
Error during patch installation :

Writing jobs to run to restart file.

AD Worker version: 11.5.0
AD Worker started at: Mon May 13 2013 17:09:43

APPL_TOP is set to /appl/RO11DV03/app


Reading jobs from FND_INSTALL_PROCESSES table ...

FAILED: file b5402149a.sql on worker 1 for product ar username AR.
FAILED: file b5081398a.sql on worker 2 for product ar username AR.

Done reading jobs from FND_INSTALL_PROCESSES table ...

Telling workers to read 'todo' restart file.
Done.
