Couldn't resolved OCI-21500 in oracle 11g

 Hi guys,
     Can you please help me to resolve OCI-21500 error ?  I am using multi threaded application which maintains connection pool. In such a scenario we are getting this error frequently although we are maintaining the connection pool mechanism in our code. We are using SourcePro C++ to get connections .

     Please advise us what to do with this. This is very urgent.
