Health & Dental deductions doubling

I have two employees whose health and dental deductions just started to double up with no changes made to their employee information or DBA instructions.  I have to process a one-time override to correct the issue.  I can't figure out what is different with the these two employees that is causing the doubling up but is not doing it on the other employees.
