Date formats in excel output are not consistent

Hi gurus,
We have designed a RTF template and using the same template to generate output in PDF and Excel formats.

The date formats in PDF are displayed correctly where as the same date formats are not consistent in excel output.

I am attaching the sample data and the RTF template along with the excel output which has inconsistent date formats.
Please rename Sample data file into .xml ( attachments are not allowed in .xml format so renamed sample data file to .txt file for uploading)

Dates are formatted in xml definition with ICX_DATE_FORMAT_MASKS  profile as per Preferences page in EBS. like to_char(Field,fnd_profile.value('ICX_DATE_FORMAT_MASK'))
