Custom Payment Format after Upgrade to 12.1.3

Hi,

We recently upgraded from 11.5.10 to 12.1.3. We have customized our check format. After the upgrade i registered the XML template and associated the template to a payment format. Also applied a couple of patches and executed a script provided  by Oracle. Even then when i make Payment i am not able to select my format.

Can someone help me?
