What is fcsm and what do fcsm errors mean?

You may encounter errors such as
fcsm(3): fc packet to DID 0xfffffa failed state: Timeout(0xc); reason: Hardware Error(0x1)

fcsm(1): ct_intr: CT command <0x118> to did 0xfffffa failed state: Packet Transport error(0xe); reason: Over run(0xc)
fcsm(0): ct_intr: CT command <0x126> to did 0xfffffa failed state: Fabric Switch Reject(0xd); reason: Unsupported Command(0xb)
fcsm(0): fc packet to DID 0xfffffa failed state: Fabric Switch Reject(0xd); reason: Unsupported Command(0xb)

fcsm is a management layer  - often used by some 3rd party software to do things like draw a diagram of the topology - not needed for normal IO traffic.
