Windows Server 2008 Best Practices to avoid loss of Axiom LUN access

Oracle Pillar Support team have seen cases where Windows 2008 server hosts lose access to their Axiom LUNs due
 to unhealthy SAN (bad SFP, cable, switch ports) and not having the latest patches and configuration from Microsoft.
