Opportunity Task not being created

Hello everyone,

I have discovered an issue regarding the creation of task through WFs.  I have WFs that create task for certain approvers(VP, Manager,CIO...) related to an opportunity

During the process the approver can accept or decline the task.  If the approver declines the task the opportunity owner can resubmit the opportunity which would start the approval process over again.

The WF that should trigger the process is not "fully" functioning.  I knowthat the condition is right for the WF since the fields on the opportunity record are being updated which is one of the actions in this WF.
