Bill to/Ship to address details are not shown in SR form

Hi

I am not able to see Bill to / Ship to address details in SR form. Already we have defined the accounts for that customer. Even though i am not able to see the address details in SR form. I am also attaching the screen shots related to this issue. Please let m eknow if you need more details regarding this issue.
