order revision history in Purchase Order JDE 9.1.3

Dear all,

Could anybody inform me how to look for the order revision history in Purchase Order JDE 9.1.3.
Actually I had no problem with JDE 9.1.1 But since we upgrade into JDE 9.1.3 the order revision's customize grids are disappear and we can't see the order revisions and make the customized grid either.

The second question is when we put some order line attachment in the PO we can see the clip signs that means that there is an order line's attachment.
But the clip sign doesn't appear in the orders Awaiting Approval - Purchase Order Requisition Approval. How to make the clip visible in the order line attachment - Purchase Order Requisition Approval.
