unable to create transaction

 hi,
When you are in TRANSACTIONS -- > TRANSACTIONS 
and then click LINE ITEMS after entering transaction data we keep 
encountering: 


FRM-40735: ON-INSERT trigger raised unhandled exception ORA-01086
