Life Insurance Plan - Premium Cost

Our Life Insurance Premium Rates have changed. Now the rate is dependent on Age and Life Insurance Amounts. Currently, our rates were determined on Age only. Does anyone have a sample of a DBA Calculation Table Set up that uses both the employee's age and Life Insurance amounts?
