Need help first time with Autopatch

Hi,
I have to apply patches Patch 10082354 and 10338643 . I have no experence with Autopatch.
Although I can find instructions about Autopatch on the web, it is not clear to me how you apply them since they can affect both the application code and the database.

I have two setups: test server has both application and database on it, the live side is split on two machines.

I need instructions on how to apply the patches in both situations and whether anything needs to be backed up, although I believe I get the idea that Autopatch does some of it's own backing up of things.
