1) While Applying patch 15843459 below form didn't generate successfully even after following readme pre-requisites & post-requisites.

The following Oracle Forms objects did not generate successfully:

ap forms/PT APXPAWKB.fmx

An error occurred while generating Oracle Forms files.
Continue as if it were successful [No] : Yes
