Creating custom XSL-XML Template for Outbound Payments

I am trying to create a custom XSL-XML template for the data definition Oracle Payments Funds Disbursement Payment Instruction Extract 1.0.  
I started out creating a custom e-text outbound payment file, attached it to a payment format, and configured a payment process profile to use it.  I successfully got a formatted payment. 

I then downloaded the seeded template Code: IBY_SEPA_CREDIT_INIT_TEMPLATE. I made my changes to the file, and created a new template exactly like the seeded one, but with a custom code and name.  I uploaded the file, changed my payment format to use the new template, and ran a payment.  The payment failed.
