12c agent does not report IP address or metrics of Windows 2008 R2 host server

I have a 12c agent installed on a Windows 2008 R2 host server. It reports to OMS 12c on a Windows 7 64-bit workstation. OMS is at the PSU2 level. In the configuration section of the host home page, it reports all values of the host except for the IP address. It also does not report any metrics for CPU, Memory, Filesystem Usage, and Network Utilization. I have re-installed the agent, clearstated it, reloaded and uploaded. All these actions have worked successfully but still no address or metrics.The agent reports on the listener and database instance just fine. The missing
